Pour le construire, l\u2019\u00e9quipe pluridisciplinaire recueille les besoins, souhaits, et centres d\u2019int\u00e9r\u00eats de la personne accueillie, de la famille et\/ou du repr\u00e9sentant l\u00e9gal.<\/p>\n<p>Le PAP d\u00e9finit les objectifs et les moyens de l\u2019accompagnement ainsi que les acteurs et les \u00e9ch\u00e9ances de mise en action.
